This policy explains what Modern Core Health & Wellness collects through this website, why, and what your choices are. It is written in plain language on purpose.
What this policy covers — and what it doesn’t
This policy covers this website only. The medical records and health information we hold as your physical therapy provider are handled separately, under the privacy practices of the clinic and the laws that apply to healthcare providers. If you are an established patient asking about your medical record, contact the clinic directly rather than relying on this page.
Please don’t send health details through this website. Our web form asks only for your name and how to reach you. Email and web forms are not secure channels. Tell us what’s going on by phone, or in your patient portal, where it belongs in your record.
What we collect
Information you give us
If you use the appointment request form, we collect your name, email address, phone number (if you provide one), and your preferred contact method. We use it for one thing: to get back to you about an appointment.
Information collected automatically
Like nearly every website, our host records standard technical information when a page is served — IP address, browser and device type, the page requested, and the date and time. These logs help keep the site running and secure. We do not use them to build a profile of you.
What we do not do
We do not run advertising trackers or analytics on this site, we do not sell or rent your information, and we do not share it with anyone for marketing.
Third-party services we use
A few parts of this site are provided by other companies. When you use them, their own privacy policies apply.
- Jane (JaneApp) — our patient portal and booking system. Clicking “Book” takes you to Jane, where any information you enter is held under their terms and our agreement with them.
- Google Maps — the maps on our Contact and Home pages are embedded from Google, which may set cookies and receive your IP address when the map loads.
- Google Fonts — our typefaces are served by Google, which receives your IP address when a font loads.
- Our web host — serves the pages and keeps the technical logs described above.
How long we keep things
Appointment requests are kept only as long as needed to respond and, if you become a patient, to connect your inquiry to your care. Server logs are kept on our host’s standard schedule.
Your choices
You can ask us what website information we hold about you, ask us to correct it, or ask us to delete it. Email info@moderncorehealth.com or call (424) 252-9424. Requests about your medical record follow a separate process — please ask us and we’ll walk you through it.
If you are a California resident, you may have additional rights regarding personal information. We honor the requests above for everyone, regardless of where you live.
Most browsers let you block or delete cookies. Blocking them may stop the embedded map from loading, but the rest of the site will work normally.
Security
This site is served over an encrypted connection (HTTPS). No website can promise perfect security, which is the reason for the note above: please keep health details out of web forms and email.
Children
This website is not directed at children under 13, and we do not knowingly collect their information through it.
Changes to this policy
If we change this policy we will update the date at the top of this page.
